PHONE CONSULTATION
- The first step toward service is a free phone consultation with an admissions representative.
- The client’s situation is discussed in detail and all questions answered.
- If the caller is comfortable moving forward, the representative gathers the client’s information and then schedules an assessment with a case manager.
- The goal of the consultation is always to be as helpful as possible.
ASSESSMENT
Even if a client is not ready to start services, a free consultation by one of our clinical
case managers can be helpful. There are always questions or issues discovered in
person that are difficult to identify over the phone.
The case manager will evaluate the client’s needs and environment to determine
what skills are needed in a caregiver.
They also discuss the wishes and expectations of the client and family in regards
to the caregiver to identify the best personality fit for the client.
If the client is ready to start services, the case manager completes the required
paperwork and creates a care plan for the client which stays in the home and is
regularly updated to ensure quality communication between case manager,
caregivers and client.
Once the admissions paperwork is complete, the case manager meets with the
scheduler to identify the best candidates from available caregivers.
SERVICES BEGIN
Services typically begin as early as the next day but same-day service can be arranged
depending on the availability of caregivers. Schedules are flexible and services can be
cancelled at any time. It is important to keep in mind that consistency in schedules helps
to ensure consistency in caregivers.
